e Perspectives on Targeted Therapy for Solid Tumors

Advances in genomics and tumor biology have led to the identification of multiple  proteins  and pathways involved in carcinogenesis and the  maintenance of the malignant phenotype, which are targets for cancer therapy.  The most studied pathways are the PI3-Kinase/Akt  and Ras-raf –MEK pathways. A number of targets have been identified in this pathway including IGF-1R, PI3-Kinase, Akt, mTOR, MEK, Raf and c-MET (Table 1). Multiple agents targeting these targets are in the clinic. In spite of the initial enthusiasm that greeted the advent of  these  targeted agents, efficacy has been modest. The few successes are with VEGF inhibitors (bevacizumab, sorafenib, sunitinib, pazopanib), EGFR inhibitors and mTOR inhibitors. In addition only a few tumor types have benefited from these agents. A number of negative studies have been performed and a number of lessons have been learnt. It has become clear that successes may be achieved with selective agents targeting protein products of actvating mutations, multitargeted agents or combinations of agents. Examples of these approaches will be discussed and c-MET inhibitors will be utilized as a paradigm.
